Ship Fire – location and source of ignition was due to friction; developed between the engaged a main drive pulley and the static or restricted boom conveyor belt. Based upon; Intrinsic analysis and our inspection of the fire scene, including the extent and location of the most extensive areas of damage due to heat throughout the scene; debris; factual observations made by eyewitnesses in closest proximity to the fires point of origin; and photographic imagery taken in time lapse duration by eyewitness’s after ignition had occurred.


Main Engine Failure – On removal, a ‘heavy grey sludge build up’ was noted on the inside of the crankcase doors.  The sludge appeared to be of water – oil emulsion. Upon inspection, it was found that there was a small piece of metal (approximately the size of a 5 cent coin) protruding from the side of one of the bottom end cap bearing shells. Main Engine was removed from service, and investigations commenced as to the cause of the failure.  Main Engine was then stripped and damaged components set aside. Found that the oil purification filters installed were inadequate and not to OEM specification.


Passenger Ship Injury [Serious Injuries]– The systemic passenger safety risks and issues were not sufficiently addressed prior to the incident involving the plaintiff . The subject vessel owners and operators were aware of the systemic risks and failed to follow the ISM Code, IMO guidelines and precautions that are consistent with the ordinary practice of good seamanship; in that they failed to control the risks to acceptable levels or eliminate the risk.


The fitted offshore Crane is a  70 Tonne Heat Compensated Hydraulics over electric, standard offshore crane assembly.  The configuration observed aboard represented the typical flow path for hydraulic fluid in a closed system. Two Main Jib Cylinders are fitted in order to operate the main jib.  Two addition knuckle boom cylinders are fitted also.

This failure related to a single, one (1) man jib cylinder. The system and equipment is configured to permit operation of either the main Jib cylinders or alternatively the knuckle jib cylinders.

 


Ship Propeller Failure – Corrosion indicated that coatings should have be applied at intervals to prevent bottom pitting corrosion that can occur in sour, foul water and/or highly corrosive environments.

Plating and associated structures are often susceptible. Maintenance inspections and the application of coatings should have been carried out before breakthrough of rust reaches 1% of the surface area.
